We are a leader in digital cognitive health assessment, partnering with healthcare providers to identify cognitive impairment earlier and streamline care workflows. Our platform allows providers to deliver efficient, validated cognitive testing with full billing and documentation support.

Position Overview:

In support of its managed services arm, we are seeking a state-licensed Nurse Practitioner (NP) to utilize their advanced clinical expertise to conduct comprehensive cognitive, functional and behavioral assessments, interpret results, and develop and implement individualized care plans in collaboration with the patient’s primary physician or specialist. The NP will engage with patients via Telehealth or in clinic for defined encounter times, document participation, and review and sign assessment reports to meet Medicare and commercial payer requirements.

This is a mainly remote, flexible, patient interaction role that combines structured patient supervision with clinical documentation responsibilities under the direction of our partner provider. Active licensure in Texas or Tennessee is required to start, with opportunities to expand into additional states as our program grows.

Key Responsibilities:

  • Utilizing BrainCheck cognitive screening tools and other assessments to fully understand the status of the patient before creating a customized care plan. 
  • Administering the BrainCheck Assess to the patient and ensuring they understood and completed the assessment based on our validity parameters. 
  • Conduct screenings for anxiety and depression
  • Conducting functional assessments of daily living activities and safety
  • Developing personalized care plans addressing cognitive symptoms, neuropsychiatric symptoms, and functional limitations.
  • Educating patients and families about cognitive disorders, including diagnosis, prognosis, and treatment options.
  • Communicating effectively with all members of the treatment team to ensure smooth transitions of care and optimal patient outcomes, following assigned provider protocols to ensure clinical quality and regulatory compliance.
  • Document supervision time and patient participation per CMS and payer guidelines

Qualifications:

  • Active, unrestricted Nurse Practitioner or Physician Assistant license in Texas or Tennessee (required).  Spanish speakers preferred.
  • Willingness and eligibility to add additional state licenses as needed.
  • Experience in geriatrics, neurology, internal medicine, primary care, or cognitive testing is a plus.
  • Strong clinical assessment skills, including the ability to conduct detailed cognitive assessments.
  • Comfortable with telehealth and digital health technologies.
  • High attention to detail and ability to meet structured documentation timelines.
  • Committed to HIPAA compliance and patient privacy.
